How Do I Apply For WIC Services

When you go to apply for WIC, please remember to bring:
- proof of identification
- proof of income
- proof of residence (where you live)
Examples Include:
Identification: (Only one is needed)
Valid driver's license, Social Security card, current work/school ID, current Medicaid card, current military ID, birth certificate, immunization record, and for infants: hospital crib card, ID bracelet or mother’s verification of facts form.
Residence: (Only one is needed)
Current utility bill, valid driver’s license, current Medicaid card (or presumptive eligibility form), bank statement, current rental or mortgage receipts, Division of Motor Vehicle ID card.
Income: (Only one is needed)
Current paycheck stubs, or recent tax return for the self-employed, current Medicaid card, a letter of certification for the NC Food and Nutrition Services, a letter from your employer stating gross income and frequency of pay, unemployment letter/notice.
To qualify for WIC, applicants must be both income eligible and have an identified medical/nutritional risk factor, which will be assessed and determined by a Nutritionist at the time of the appointment..
With some exceptions, each person applying for WIC must be physically present at the time of application at the local WIC office.
How Do I Change My Infant’s Formula?
1. Determine if a prescription will be required.
No Prescription Required:
Formulas that do not require written authorization or a prescription from the primary health care provider includes:
Enfamil Premium Newborn
Enfamil Premium Infant
Enfamil Prosobee &
Enfamil Gentlease.
Parents may simply come in and request their infant’s formula be changed. We recommend that this be done only after the primary health care provider has knowledge that the current formula is not being well-tolerated by the infant and after the infant has sampled one of the other formula products and has tolerated better.
Prescription Required:
Formulas that require written authorization or a prescription from the primary health care provider includes:
Enfamil AR
Similac Expert Care Alimentum
Enfamil Nutramigen
Enfamil Enfacare
Enfamil Premature
Neocate
Before coming to your local WIC office:
- Check to see if the formula that the infant is changing to requires a prescription.
- If a prescription is required, make sure the prescription is written on the NC WIC Program Prescription form {DHHS 3835}. Most pediatricians have access to the required form.
- Prescriptions must be complete with duration, medical condition(s) indicating the need for the prescribed product, date, and provider’s signature signature. Incomplete prescriptions are not accepted.
Process:
- When an infant’s formula needs to be changed, the parent/ guardian must bring the WIC wallet, the prescription {if required- refer to part 1 to see if the desired formula requires a prescription}, any current/future WIC formula vouchers, and any unopened formula the infant may have at home.
- WIC staff will change the infant’s food package as requested by the parent/guardian or prescribed by the primary health care provider.
- Prescriptions are honored for the duration provided by the primary health care provider only, and often requires subsequent prescriptions.
- It is necessary to know the duration of a prescription to ensure a subsequent prescription is obtained when required if necessary.
Can I Still Receive WIC Services After I Delivery?
If a postpartum mother meets the three qualifications of the WIC program {resides in Richmond County, is income eligible, and has a nutritional risk}, she continues to be eligible to receive WIC services until her infant is six months old.
If a postpartum mother is partially or exclusively breastfeeding and meets the three qualifications of the WIC program, she continues to be eligible to receive WIC services up to 12 months postpartum or as soon as she discontinues breastfeeding after month 6.
How Do I Add My Infant On The WIC Program?
The process of adding an infant on the WIC program is greatly simplified if the mother is already a WIC participant during the pregnancy. Healthy newborns should be present at the initial certification. Please refer to question 1; " How Do I Apply For WIC Services"?
Are Breastfeeding Mothers Given Any Extra Incentives?
Breastfeeding is the optimal method of infant feeding and is strongly encouraged. In an effort to encourage, protect, and promote breastfeeding, the Richmond County WIC program various methods of support including:
- Provide monthly breastfeeding classes (group setting or individualized).
- Enhanced WIC food package {for mom & infant at 6 months}.
- Extension of WIC services for mom for up to 12 months postpartum.
- Continued breastfeeding support and encouragement through our Breastfeeding Peer Counselor Program.
- Breastfeeding Supplies including double pumping electric breast pump.
- Breastfeeding literature or information.
- Access to a designated telephone line for breastfeeding support or to schedule an appointment.
- Access to Latch On Headquarters, the newly renovated breastfeeding room located on the WIC hall inside of the health department.
